About the community of practice

The Frailty Care Community of Practice aims to help you improve your knowledge and practice around frailty and palliative care.

The 11 hospices who took part in our Extending Frailty Care programme will share their learning so that others can adapt, replicate and scale up their models of care.

We will also hear from subject matter experts, who will share their expertise and discuss key issues around frailty care.

The Frailty Care Community of Practice uses the Project ECHO learning methodology. It is led by Cat Sullivan, Senior Clinical Practice & QI Manager, Hospice UK.

The community of practice will meet once a month, for six months, via MS Teams.

Members of the community of practice will be added to a MS Teams discussion group, where you can share ideas and resources between meetings.
