On behalf of the hospice I represent, I confirm that by signing up to the National Hospice Legacy Campaign now:

- the proposals for setting up the Campaign as set out in the information (including the FAQs) provided by Hospice UK on its website are understood and agreed and,

- my hospice will pay its contribution to the Campaign calculated by reference to its annual expenditure (as evidenced by its last filed statutory accounts as at the date of signing up) on a date on or before 30 April 2024 provided the hospice has first received a VAT invoice for this contribution from Hospice UK.
